The One Direction star, 29, was supporting former bandmate Louis Tomlinson at the premiere of his new film All Of Those Voices when he surprised with his red carpet appearance.
Fans flocked to Twitter to question what had happened, with many asking if he'd undergone buccal fat removal.
The fast growing trend among celebrities and social media personalities alike cuts out fat pads from the inside of a person's cheek, giving them a more angular face as a result.
Shocked fans penned: 'Liam Payne has had a full face job , respect it'; 'Liam Payne getting buccal fat removal is sending me to the actual moon'; 'Still can’t believe Liam Payne got buccal fat removal.'
Speaking to MailOnline, aesthetics doctor Dr Mo Hamed weighed in on what he believed Liam could have had done, saying the singer was looking 'very good.'
Dr Mo Hamed, who runs aesthetics clinic Dr Motox, speculated: 'He looks like he may have had some jawline filler to give him a chiselled and contoured side profile.
'This is becoming a very popular treatment in my clinic where men are having jawline filler to masculinise their face.
'A wider and chiseled jawline is associated with more strong masculine features.'
MailOnline has contacted Liam's representatives for comment.
Speaking in 2021, Liam revealed he was left looking bloated after going through a 'wild booze and pills phase'.
Liam, who said he struggled to stay sober on tour, revealed: 'There were a few pictures of me on a boat and I was all bloated out and I call it my pills-and-booze face.
'My face was 10 times bigger than it is now. The problem was, the best way to secure us was to lock us in our rooms, and what is in the room? A mini-bar.
The procedure cuts out fat pads from the inside of a person's cheek, giving them a more angular face as a result.
Model Chrissy Teigen admitted to receiving the procedure in 2021, and it just one of many personalities believed to have underwent the operation.
It takes around a half-hour for the surgeon to complete each side of the mouth, with the entire operation taking around an hour.
The surgeon will then close the incisions with dissolvable stitches. Payne’s jawline appeared sharper than it has in the past, and his cheekbones were also more defined.
Dressed in a white blazer with black accents and a pair of black pants, he posed for photos with his new girlfriend, Kate Cassidy, but social media users paid more attention to the former boy bander’s looks than his date.
“Omggggggg what happened to Liam?” one person commented under an Instagram post of Payne, 29, and Cassidy, 23, on the red carpet.
“Liam and his girlfriend probably have their dates [at] a beauty clinic, so they can have the opportunity to get a s–tload of fillers and Botox together 😂,” another quipped.
“Did Liam also got [sic] on the buccal fat removal trend????” someone else asked, referring to a plastic surgery popular among celebrities like Chrissy Teigen that helps thin the cheek area.
“So good to see them together. Liam looks really smart, but so thin. His face is gaunt,” one alarmed fan tweeted.
However, many Directioners rushed to Payne’s defense, calling him “hot” and “beautiful as always.” Others pointed out that the “Bedroom Floor” crooner previously described himself as “bloated” while struggling with alcohol and pills, which he has since kicked.
