A clip surfaced on Tiktok of Katy on stage in Las Vegas struggling to open her right eye.
The singer can be seen trying to prompt it open with her hand, and appears to be struggling to lift her eyelid.
As her eye flickers, Katy is staring off into the distance in the middle of her show.
The mum then turns to look at the crowd and regains composure, before telling fans: "Make even more noise for my band."
But, the moment saw many fear the 38-year-old had suffered from facial paralysis.
One concerned user wrote: "What in the Justin Bieber is going on here?"
Another added: "Is this real? Really worried for her."
And a third chimed in: "Doesn’t matter what caused it. The fact that it happened is scary as hell.
Amid all the controversy surrounding Katy's eye flicker, hardcore fans were sure to set the record straight.
They explained the California-born artist takes on the roll of a doll-like character as a skit throughout her show.
One Reddit user observed: "It's like one of those dolls that you tilt back and the eyes close, but one of the eyelids got stuck open.
While the show went on, fans have been left digging around for answers as to what went on in those brief few seconds.
“BELL’S PALSY???” one person commented, referring to the facial paralysis condition.
“Maybe her eyelashes are stuck?” another suggested, while one added that it could be due to stress. Another wrote: “She looks possessed!”
However, taking to Reddit, diehard KatyCats (the name for Katy Perry fans) claim it’s actually all part of her act.
“OK so this is some weird gag she does at her show to resemble those old dolls that opened and closed their eyes when you tilted them,” a fan explained on Reddit.
“I was kinda worried it was a stroke or surgery mishap. But no, she does that on purpose.”
Another weighed: “ Yeah she does it pretty frequently during the show. Not every night but she’s done it at least once per 8 show run.”
Meanwhile, another fan, who said she’d spoken to a make-up artist friend, had a much more practical theory as to what was causing Katy’s eye to malfunction.
“Katy has false lashes on in this. It’s likely there’s something in her eye, or bothering her eye, but she can’t rub them because it will ruin her makeup,” she claimed.
“Stage makeup is very heavy and will get everywhere when you touch it. She’s gentling pulling her temple area to try and alleviate the itch or clear whatever is bothering her eye without doing damage.”
Whatever the cause, Katy has actually spoken of what she described as her “wonk eye” before, when she was a judge on American Idol.
“A bunch of my fans created a fandom over my wonk eye. I even have a fandom that calls itself, ‘Katy’s wonk eye’,” she said.
“It’s my right eye. So just celebrate all that stuff. Don’t worry about it,” she told a contestant with the same issue.
Although Katy has yet to confirm exactly what was going on with her eye, she previously revealed in 2011 that she had a prescription for her eye condition, although it’s unclear if this is what caused her eyelid to close on stage.
